You are a foundation doctor working in Happy GP Surgery. 27-year-old Sammy Wong has come in complaining of weight gain and tiredness. Take a full history from Sammy. Please answer the examiner's questions at 7 minutes.

PATIENT INSTRUCTIONS

  • I am so tired everyday….’

    • Onset: Has been feeling tired for the past 6 months. Always thought it was due to my job nature of working from 9-5 everyday, but even on weekends, I have no energy to socialise. I have stopped hanging out with my friends. I just feel very tired everyday.

    • Progression: I think my energy levels have been low for the past few months. I have also noticed a significant weight gain of 7kg over the past 6 months although my appetite has reduced. 

    • Mental state: I have not been feeling great.

  • only mention when asked each question specifically: 

    ‘Have you been feeling depressed/hopeless for the past few months?’ - yes, at times 

    ‘Have you lost interest/pleasure in doing things you enjoy?’ - yes. I used to enjoy playing the piano but I have not had any energy to play for the past few months. 

    ‘Have you experienced any trouble concentrating at work?’ - no

    ‘It is my responsibility to also ask this question whenever my patients present with mood changes. Do you have any thoughts of life not worth living for the past few months?’ - no 


    • Ideas: I have no idea why this is happening.

    • Expectations: I really want to find out the cause. 

    • Concerns: ‘Would this be cancer, doctor?’


    • Temperature tolerance: I now need to wear a thick cardigan at work although my colleagues think it's warm in the office. 

    • Bowel habit: Occasionally I suffer from constipation. I have tried to increase my fibre intake. 

    • Menstruation: My cycles are normal. I do suffer from heavy menstrual bleeding ever since puberty. 

    • Sleep quality: at least 7 hours of sleep everyday. My sleep quality is good. 

    • Palpitations: no

    • Biotin intake: I do not take any extra biotin supplements.

  • Medical conditions: none

    Previous surgeries: none 

    Regular medications: only multivitamins everyday

    Over the counter medication: none 

    Excessive biotin intake: no

    Drug allergies: I am allergic to penicillin.


  • My mother has a thyroid gland issue. I am not sure about the details.

    • Smoking: I have never smoked. 

    • Alcohol: I don’t drink at all. 

    • Recreational drugs: never

    • Diet: I am trying to increase my fibre intake.

    • Exercise: no

    • Home: I live alone.

    • Occupation: Accountant

    • Impact of symptoms on activities at home/work: My tiredness and weight gain have both decreased my confidence at work. I am now shy to speak to my colleagues because of my appearance. The first thing they always say is ‘You have gained so much weight!’
