You are a junior doctor working in GP. Mr Jacob White presents to the practice with acute blurring/ worsening of vision.

EXAMINER INSTRUCTIONS

Timing Instructions: At 8 minutes, stop the candidate and ask the following questions:

  • Answer 1: Retinal detachment, Vitreous haemorrhage, retinal artery occlusion, retinal vein occlusion, stroke (any reasonable cause of acute loss of vision)

  • Answer 2: Retinal detachment

  • Answer 3: Diabetic retinopathy, family history, trauma, posterior vitreous detachment

  • Answer 4: Vitrectomy, scleral buckle, pneumatic retinopexy

  • Retinal Detachment

  • Name: Jacob White

    Age: 56 years old

    Date of Birth: 1st December 1968

    Location: Lay’s Surgery

    Occupation: Investment Banker

  • "I noticed my vision becoming blurry on the way to work this morning."

    • Site: Only my right eye. Which is also my better eye!

    • Onset: Started this morning when I was on the subway to work

    • Character: It was very sudden, without any warning. Felt like a curtain closing on my eye (peripheral vision loss). No pain

    • Precipitant: No trauma or injury

    • Assoc Sx:

      • No redness

      • No discharge or watering

      • Both near and far vision affected

      • Noticed some floaters in my right eye

    • Exacerbating Factors: None

    • Relieving Factors: None

    • Recent illness: None.

    • Recent travel: None.

    • Previous episodes: Never experienced this before.

    • Ideas: "Do you think I’ve got an infection?"

    • Expectations: “Is there something that can be done?"

    • Concerns: “My eyes are very important for my work! It won’t be permanent will it?"

    • Bowel habits: Nil

    • Urinary habits: Nil

    • Appetite: Same as usual.

    • Sleep: Same as usual

    • Weight: No change 

    • Pain in the chest/abdomen: None

    • Headaches: None

    • Fever: None

    • Cardiology: Normal

    • Respiratory: Normal

    • Upper limb neurology: None

  • Previous hospital admissions: None

    Previous surgeries: None

    Medical conditions:

    • T2DM

    Medications:

    • Metformin

    Allergies:

    • None.

  • My grandmother had to undergo some eye operation when she was younger but I’m not sure what for

    • Smoking: Never

    • Alcohol: Occasional can of beer on the weekends

    • Recreational drugs: Never

    • Diet: I cook most of my meals

    • Exercise: I try to go on runs everyday before work

    • Ill contacts: No one around me was recently ill

    • Home: Lives with wife and daughter in an apartment

    • Active Daily Living: Independent with all ADLs

    • Occupation: I work as an investment banker
